FP Canada requires a comprehensive physical, psychoeducational, and/or neuropsychological evaluation from a doctor and/or qualified health practitioner whose credentials are appropriate to diagnose and treat your disability or unique accessibility need.
Please note that a diagnosis for accommodations related to comprehension and processing of information is not sufficient by itself. Documentation of supporting testing is required.
This documentation must be current, (i.e., within the last five years), be printed on your healthcare provider's office letterhead and include the following in detail:
- a clear diagnosis and a description of your medical or health issue
- how this diagnosis affects your ability to take the exam under standard conditions
- results of any assessments related to your request
- specific details of the accommodations requested
There are additional requirements for submissions for accommodations related to a diagnosis that impacts comprehension and processing of information.
For these diagnoses, a family doctor is not the expected respondent. An appropriate respondent in these instances would be a specialist in mental health, such as a psychiatrist, or psychologist supported by a recent psychosocial or psychoeducational assessment (i.e., within the last five years).
FP Canada reviews and grants all accommodation requests on a case-by-case basis:
- submitting a request does not guarantee it will be granted
- if you do not include the required information, your request will be returned
- reviews can take up to 30 days to complete
- FP Canada cannot guarantee that requests received less than two months prior to the exam date will be assessed or granted
- confidentiality of all submissions is maintained throughout the process
If your accommodations are granted, we will send you a letter to review and sign in agreement.
